Our Onboarding Process
STEP 1: Complete NAF
Complete our New Account Form (NAF), which collects basic practice, sample, logistics, and provider information.
STEP 2: Attach NAF
In the field below, please fill out the following form, and attach the signed & completed New Account Form.
STEP 3: New Account Creation
Upon receiving your New Account Form our specialists will build your account, and provide you with your unique portal logins, along with training.
